You will be asked to complete the Patient Sign-In Sheet, Informed Consent, and Financial Policy Forms. This process usually takes 10-15 minutes.
To help reduce the amount of time completing paperwork during your first visit, you can complete the following forms at home and provide them to our receptionist when you arrive for your appointment.
To uproot the underlying cause of your symptoms, the doctor will take a detailed history pertaining to your health concerns and/or problems followed by the exam, treatment options, and goals.
A diagnostic chiropractic exam is performed, which encompasses an orthopedic test, a neuromusculoskeletal test, functional ranges of motion are tested and a therapeutic home strengthening program is discussed and provided. These tests and exercises are specific to your chief complaint.
Imaging deemed necessary at the time of the visit, will be referred out to the local digital imaging center. Additional charges will apply for these services, which are not included in your new patient exam fee.
Treatment options will be discussed and provided. These options may include: spine and/or extremity adjustments, soft tissue mobilization, physiotherapy, or Kinesio® tape applications.
Strengthening and stretching exercises will be demonstrated and provided to the patient.
To ensure the patient is responding to treatment in a positive manner, you will likely schedule a follow-up visit within 2-3 days. The duration between follow-up visits will be modified as the patient progresses through their treatment plan.
